Abstract:

Introduction: 60 percent of hospitalization causes of intravenous drug users are infection, of which 5 to 15 percent of them due to endocorditis. Infective endocorditis is a common infection among intravenous drug users and can cause high mortality and morbidity. This study was conducted to evaluate clinical and para-clinical presentations of drug users. Methods: In a descriptive study we evaluate 33 narcotic addicts with documented infective endocarditis in two tertiary care hospitals in Tehran, Iran Imam Hosein and Loghman Hakim hospitals during 2002-2008. Demographic information, clinical manifestation, radiologic, laboratory and echocardiographic data were evaluated. The data were analysed using SPSS software. Results: 82% of patients were in 20-40 year old age group. 52% of the cases were referred to the hospital with main complain of fever and cough and 27% with pain and fever. Fever was observed in 91% of the patients. Positive blood culture was observed in 9 cases and negative blood culture in 14 cases. The most common involved valve was tricuspid. Mortality rate was 45%. Conclusion: Endocarditis should be considered in intravenous drug users when they present with fever. Blood culture may be negative due to previous use of antibiotics and poor technique of culture.

Clinical and molecular epidemiology of infective endocarditis in intravenous drug users.

BACKGROUND Infective endocarditis (IE) in intravenous drug users has been increasing in incidence. The major pathogen used to be methicillin-susceptible Staphylococcus aureus, but resistant isolates have also been increasing. This study aimed to investigate the clinical characteristics of IE in intravenous drug users and to evaluate the molecular patterns of methicillin-resistant S. aureus (MRS...

effect of vegetation size on the outcome of infective endocarditis in intravenous drug users

background infective endocarditis is one of the most important complications of injection drug use, which is associated with significant mortality and morbidity. the aim of this study was to evaluate the influence of vegetation size and localization on outcome and in-hospital mortality of infective endocarditis. patients and methods a total of 34 patients were admitted to a university hospital ...

Clinical Features of Right-Sided Infective Endocarditis Occurring in Non-Drug Users

Right-sided infective endocarditis (RIE) occurs predominantly in intravenous drug users in western countries, and it has a relatively good prognosis. Clinical features and prognosis of RIE occurring in non-drug users are not well known. We investigated the clinical findings of RIE in non-drug users. We retrospectively reviewed 345 cases diagnosed with IE. Cases with RIE or left-sided infective ...

Surgical treatment of infective endocarditis in active intravenous drug users: a justified procedure?

BACKGROUND Infective endocarditis is a life threatening complication of intravenous drug abuse, which continues to be a major burden with inadequately characterised long-term outcomes. We reviewed our institutional experience of surgical treatment of infective endocarditis in active intravenous drug abusers with the aim of identifying the determinants long-term outcome of this distinct subgroup...

Clinical and economic outcomes of infective endocarditis.

BACKGROUND In France, the estimated annual incidence of infective endocarditis (IE) is 33.8 cases per million residents. Valvular surgery is frequently undergone. We report an epidemiological and economic study of IE for 2007-2009 in a French region, using the hospital discharge database (HDD). METHODS The population studied concerned all the patients living in Centre region, France, hospital...

Clinical significance of hyperhomocysteinemia in infective endocarditis

Blood coagulation plays a key role in the pathogenesis of infective endocarditis (IE). Conditions associated with thrombophilia could enhance IE vegetation formation and promote embolic complications. In this study, we assessed prevalence, correlates, and clinical consequences of hyper-homocysteinemia (h-Hcy) in IE.Homocysteine (Hcy) plasma levels were studied in 246 IE patients and 258 valvular...
